Phoenix Place is a registered provider of NDIS supports. We are committed to creating a more equitable and inclusive society for people with disabilities. We provide person centered supports to over 200 participants in the Mackay Region.

Our friendly and knowledgeable support staff will prioritize your goals and needs, ensuring that you have the choice and control to shape your support experience. Our programs are designed to respond to the unique needs and strengths of our participants, built on over two decades of experience serving the Mackay community.

What we do.

SUPPORT COORDINATION

Assistance navigating NDIS plans.
Helping you choose & connect with service providers that meet your individual needs.
Assistance exploring and coordinating with community and mainstream services.
Help with developing goals for your plan and assistance with re-evaluating throughout the year.
Support in identifying and connecting with opportunities to increase your confidence and build capacity for greater independence.

SUPPORTED INDEPENDENT LIVING

Daily Living Support: Assistance with activities of daily living (ADLs) such as cooking, cleaning, and personal care.
Transportation: Help you get to and from events, appointments, & activities.
Skill Building: Help you develop & improve your skills & capacity - cooking, budgeting, and communication, whatever your goals may be.
Social Support: Support to help you participate in social activities and connect with your community.

SOCIAL & COMMUNITY GROUPS

Adult Social & Community Groups including Craft, Bowling, Fishing, Cooking, Yoga, Technology, Smart Buys, Friday night out

We also facilitate Youth Before/After School Programs and School Holiday Programs.

Each of our groups are focused on promoting fun, independence and community engagement.

SKILLS BUILDING & PATHWAY PROGRAMS

We currently facilitate a Hospitality Pathways program in our Deja Brew Cafe.
